Please have the following available and requirements met at the time of collection:
• Valid government-issued photo ID for the participating adult
• Pregnancy must meet the laboratory’s minimum gestational-age requirement for testing
• The mother will need to provide the required blood sample
• Please provide any pregnancy-related information or documentation requested when your appointment is confirmed

Your appointment includes mobile DNA collection within our standard service area. Additional travel charges may apply to locations outside the included service area. Any additional mileage charge will be confirmed with you before your appointment.
Cancellation Policy: Same-day cancellations are subject to a $75 cancellation fee. Once DNA samples have been collected and/or laboratory processing has begun, payments are non-refundable. Please contact us as soon as possible if you need to reschedule or cancel your appointment.
